Order By Fax

1. Look up the items you need in our product catalog or online.  If you do not currently have the new 25th Anniversary 2004-2005 Catalog, we can mail you one if you fill out the form at:

2. To find pricing of the items in your order, look up items by part number at: http://www.uvprocess.com/pricing/
    
3. For your convenience, we have an order form you can print and fill out at:
   http://www.uvprocess.com/Order/faxform.asp
   For each item, we will need the following information: Part Number, Quantity, Size (if applicable), Description, Price Each, and Total Price.
    
4. Upon ordering we will need your Shipping address, contact person, and phone as well as a Billing address, contact person, and phone.  If you have an account with us, we will also need your purchase order number.  If you want to pay with credit card, please include all requested information in the credit card section. If you prefer to pay by COD, you can make such arrangements when you call your order in.
    
5. You may fax in your order 24 hours a day at 1-800-99FAXUV (800-993-2988) if you are located within the US or 1-011-773-880-6647 if faxing from outside of the US.
    
6. If you have any questions, please call us at 800-621-1296 if you are located within the US or 1-011-773-248-0099 if calling from outside of the US.  Our normal business hours are between 8:00AM and 5:00PM CST.  We are closed on weekends and all federal holidays.
